Rules & Regulations
Registration Deadline: September 19, 2008
Entry Fee is $1,600.00 per four-player team.
Tournament Committee decisions are final.
Company Eligibility:

Bona fide businesses are eligible—Federal Tax ID will be required on Official Entry Form. |
Player Eligibility:
| • |
Amateur players only—No Golf Professionals. |
| • |
Players must be at least 21 years old. |
| • |
Players must have at least one-year employment with team company. |
| • |
Players must have an established, valid and current USGA approved handicap index — All handicaps must be computed by the GHIN computation service. |
| • |
Handicaps not computed by GHIN will compete with a “0.0” handicap index. |
To Form a Team:

Total maximum handicap index of all 4 amateurs combined will be no more than 50.0. If total handicap is greater than 50.0, the player with the highest handicap index may play off a lower handicap to make the team eligible. |
Subscription:

Limited to first 18 Teams with paid registration. |
| • |
Over-subscription will result in a Priority List, withdrawals being filled from said list. |
| • |
Under-subscription may result in cancellation of event and refund of entry & sponsorship fees. |
Tournament Entry & Withdrawals:

Entry Fee includes use of range, golf cart, golf & awards banquet for one 4-player team. |

Entry Fees payable by check, M.O. or credit card. |
| • |
Withdrawals must be made on or before 9/19/08. |

Players submitted on Official Entry Form must be present to play. Changes to team roster must be made prior to 9/19/08 deadline. Contact Tournament Manager to make player changes. |

Refunds are subject to a 10% charge if made within 2 weeks of event. |
